Your health record contains personal information about you and your health. State and federal laws protect the confidentiality of that information. “Protected Health Information” (“PHI”) refers to any information that identifies you and relates to your past, present, or future physical or mental health condition, substance use, or healthcare services.
The confidentiality of alcohol and drug treatment records is specifically protected under 42 C.F.R. Part 2 and the Health Insurance Portability and Accountability Act of 1996 (HIPAA). These combined laws restrict most disclosures without your written authorization. Exceptions apply only in specific limited circumstances—for example, for medical emergencies or in response to a court order issued under the required procedures. Any violation of these laws is a serious matter and may be reported to the appropriate authorities.
If at any time you believe your privacy rights have been violated, you have the right to file a complaint (see the “Complaints” section below).
For Treatment
We may use and disclose your medical information to provide, coordinate, or manage your treatment and related services. This can include communication with healthcare professionals, laboratories, or referral sources involved in your care.
For Payment
With your authorization, we may use and disclose your PHI to obtain payment for services rendered. Examples include verifying insurance benefits, processing claims, or coordinating reimbursement with government programs or other payors.
For Healthcare Operations
We may use your PHI as needed for our internal operations—such as quality assessment, staff training, accreditation, licensing, audits, and compliance monitoring. For example, we may use a sign-in sheet or call your name in a waiting area. Business associates performing administrative functions (billing, data processing, etc.) are required by contract to safeguard your PHI and maintain confidentiality.

Right to an Accounting of Disclosures
You may request an accounting of certain disclosures of your PHI made during the past six years, excluding those made for treatment, payment, or operations, or disclosures authorized by you. Reasonable fees may apply for more than one request in a 12-month period.

You have the right to request restrictions on how we use or disclose your PHI for treatment, payment, or operations. We are not required to agree to a requested restriction, but if we do, we will comply with it.

You may request a printed copy of this Notice at any time, even if you originally received it electronically.
Right to Receive a Copy of Revised Notices
We reserve the right to revise this Notice and to make the new Notice effective for all PHI we maintain. Any updates will be posted on our website, provided upon request, and available at our facility.
